What you'll do

• Provide direct resident care and distribute medications according to individualized care plans
• perform assessments as required by state standards and facility policy
• observe, document, and communicate changes in residents’ conditions
• maintain care plans and attend interdisciplinary meetings
• record vital signs, weights, and blood sugars as assigned
• support activities of daily living, restorative nursing, behavioral support, and fall prevention
• oversee medication orders, MAR documentation, narcotic counts, and pharmacy coordination
• assist with caregiver orientation and coaching.
