The case manager coordinates care, advocacy, and treatment planning for residents from enrollment through discharge. The role works with residents, families, guardians, and treatment teams to support individualized goals, safety, education, and independent living skills.

What you'll do

• Develop and implement treatment plans, Positive Behavior Support Plans, and IEPs with residents and treatment teams
• advocate for residents’ treatment, behavioral, safety, and health needs
• coordinate family communication and team meetings
• maintain and distribute plans and reports
• participate in admissions, discharge, and aftercare processes
• complete assessments and support crisis intervention when available.
