Care Coordinator

What you'll do

• Process patient referrals
• answer incoming calls and assist patients, providers, and external offices
• obtain consult notes, imaging reports, and other documentation
• coordinate communication among providers, patients, and outside facilities
• maintain patient records in the electronic medical record system
• ensure referrals are completed promptly
• protect patient confidentiality
• perform additional administrative and care coordination duties as assigned.
