What you'll do

• Help patients navigate healthcare services and access resources
• identify barriers to care and connect patients with community and social services
• support appointment scheduling, referrals, follow-up care, and service coordination
• explain available healthcare programs
• communicate with clinical staff and community organizations
• document patient interactions and services
• and protect confidentiality in accordance with HIPAA and organizational policies.
