What you'll do

• Re-evaluate patient nursing needs

• use assessment data to determine nursing diagnoses
• develop care plans with patients and families

• counsel and instruct patients and families
• identify and implement discharge planning needs
• prepare clinical notes and update physicians
• coordinate with community health-related contacts
• participate in on-call duties as defined by policy
• ensure necessary equipment and services are arranged.
