Support Plan Coordinator

Job overview

The Support Plan Coordinator role is onsite in Colonial Heights, VA, with pay of $25 per hour. Jerolin Management Services’ community-based programs serve people with intellectual disabilities through waiver services. The coordinator oversees program expansion and service delivery, supervises and trains staff, and prepares support plans and reviews. The role coordinates with individuals, agencies, and departments as needs change, while maintaining required state-system records and authorizations.

What you'll do

• Monitor and supervise staff, including approving timesheets
• train staff on program duties
• prepare staff schedules, Individual Support Plans, and Person-Centered Reviews
• participate in planning meetings
• enter serious incidents and allegations in the CHRIS system
• submit Service Authorizations and supporting information in WAMS
• coordinate service delivery with other agencies and departments.

Why this role

Experience with individuals with intellectual disabilities and interpreting Individual Support Plans is highly preferred. A human services degree is preferred, or five years of relevant plan-completion and staff-supervision experience.
