What you'll do

• Conduct and coordinate resident assessments and interviews
• evaluate resident conditions and medical information
• ensure assessments and care plans are completed within required timeframes
• guide assessment team members on accuracy and completion
• participate in interdisciplinary care plan and family meetings
• maintain accurate MDS coding and records
• identify and correct coding errors
• transmit reports to Medicare and Medicaid databases
• develop and review care plans
• prepare plans of correction for survey deficiencies
• support discharge planning and respond to resident concerns.
