The Registered Nurse (RN) Care Coordinator role is onsite in Griffin, GA, with a variable shift; compensation is not specified. The role coordinates care facilitation, progression, and transitions so patients’ clinical and psychosocial needs are addressed and discharge plans move forward efficiently. The nurse works with patients, families, physicians, utilization review, and the interdisciplinary team to identify barriers, arrange appropriate transitions, and document care planning.

What you'll do

• Assess patients’ chronic disease management needs, psychosocial risks, and available discharge resources
• coordinate individualized discharge planning with patients, families, physicians, care teams, and payers
• participate in interdisciplinary rounds and post-acute referrals
• identify and escalate barriers or delays in care progression
• document assessments, interactions, services, and discharge plans
• track avoidable days and report trends.
