Hospice Certified Nursing Assistant (CNA)

The Hospice Certified Nursing Assistant (CNA) role is field-based in Town of Grand Chute, WI, with visits to patients in private residences, assisted living, or care facilities. Pay is $18–$23 per hour. Moments Hospice supports patients’ comfort and dignity and provides support to their families. Working with an RN Case Manager and the interdisciplinary care team, the CNA provides hands-on personal care and follows each patient’s plan of care. The role helps patients remain comfortable and cared for wherever they call home while supporting families through hospice care.

What you'll do

• Provide personal care including bathing, dressing, grooming, toileting, and skin care
• assist with lifting, transfers, positioning, range of motion, exercise, ambulation, dining, and nutrition
• follow each patient’s plan of care under the RN Case Manager’s direction
• provide comfort and companionship and support families
• maintain electronic medical records.

Why this role

Active CNA certification and a valid driver’s license with current auto insurance are required. The schedule is Monday–Friday, 8:30 AM–5:00 PM. A company vehicle program is offered subject to eligibility.
