What you'll do

• Maintain an assigned caseload
• develop, prepare, and monitor individual support plans
• coordinate appointments and meetings
• support individuals and families and liaise with agencies
• coordinate prospective admissions and process discharges
• conduct unannounced visits as needed
• ensure paperwork is accurate and timely
• assist with program development and community relations
• support staff training and oversee assigned staff
• monitor administrative staff timesheets, mileage, and schedules
• assist in homes as needed for staff coverage.
