What you'll do

• Conduct psychosocial assessments and identify social, emotional, financial, and environmental needs
• provide social work services in acute care/medical-surgical settings
• develop discharge and transition-of-care plans
• address discharge barriers including transportation, housing, finances, caregiver support, and follow-up services
• provide emotional support, counseling, advocacy, and crisis intervention as appropriate
• connect patients and families with community and healthcare resources
• collaborate with the multidisciplinary team
• participate in care conferences
• document in the electronic medical record
• monitor needs and adjust interventions
• manage multiple cases and prioritize urgent needs.
