The Foster Care Case Manager role is onsite in Morganton, NC, with pay of $40,800–$48,545 per year. A $2,000 sign-on bonus is available to qualified candidates. Children’s Hope Alliance works to support children and families through pathways to hope, health, and healing. The case manager coordinates care, services, and treatment planning while advocating for children and supporting foster families. The role helps promote safe, nurturing placements through family collaboration, home visits, documentation, licensing compliance, and crisis/on-call support.

What you'll do

• Guide children, foster families, and care teams through the foster care journey
• coordinate services, treatment planning, and resources
• facilitate Child and Family Team Meetings and contribute to admission, treatment, and discharge planning
• coach and train foster parents
• conduct in-home visits
• support families with behavioral challenges using models such as the Teaching Family Model
• communicate with biological families, social workers, and community partners
• represent the organization in court hearings and reviews
• maintain case records and monitor foster home licensing compliance
• review medical administration records
• participate in on-call rotation.
