The Clinical Mental Health Professional - Addiction Medicine is an onsite role based in Grand Junction, CO, with compensation ranging from $35.60 to $54.93 per hour. This position is part of the Intermountain Health team, which provides integrated medical and behavioral health services for individuals and families affected by addiction and co-occurring mental health conditions. This role exists to provide behavioral health assessment, intervention, and case management. The professional will work within an interdisciplinary team to develop and implement care plans, provide crisis intervention, and connect patients with necessary community resources to ensure continuity of care.

What you'll do

• Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan(s) to reflect changes in patient status
• Refers patients to other services to improve their well-being, including community services, treatment programs, support groups, psychiatrists, or other mental health providers
• Works with the multi-disciplinary team to develop a discharge plan to ensure continuity of care
• Organizes and prioritizes daily work by assessing new, current, and discharging patient needs
• Performs behavioral health consultation for patients and caregivers
• Conducts psychosocial assessments
• Provides supportive, crisis bereavement and other behavioral health interventions as necessary
• Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care and timely discharge
• May provide support and debriefing for staff members following adverse events.
