What you'll do

• Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement therapeutic interventions focusing on functional strengthening, gait training, balance, and fall risk reduction.
• Manage caseload and schedule home visits based on availability and patient needs.
