What you'll do

• Develop and maintain relationships with physicians, hospital discharge planners, case managers, nursing facilities, and community organizations
• educate referral sources and the community about hospice care
• conduct in-person outreach, presentations, and networking
• collaborate with the clinical team on patient transitions
• work toward referral and admission goals
• maintain outreach and referral records in CRM software
• monitor industry and regulatory developments
• and comply with laws governing agency promotion.
