Social Worker — Hospice PRN

What you'll do

• Prepare social histories and contribute to care provider plans
• provide social casework, short-term individual counseling, community resource planning, and crisis intervention when necessary
• assess psychosocial status and family financial situations when appropriate
• refer patients and families to community agencies
• participate in agency and interagency case conferences
• maintain clinical and progress notes
• coordinate with physicians and other disciplines on discharge planning.
