The case manager provides psychological and behavioral case management to people using crisis services, supporting stabilization and transition back to the community. The role assesses needs, helps people access basic resources and services, supports daily living and recovery goals, and documents consumer interactions.

What you'll do

• Support consumers with basic needs and benefit applications
• arrange transportation at discharge
• provide psychosocial services and daily living guidance
• support employment, housing, and community-resource access
• engage natural support systems
• educate consumers about services
• maintain a trauma-informed environment
• document interactions.
