The dietitian assesses patients, develops nutrition care plans, provides medical nutrition therapy and counseling, and supports behavior change and long-term lifestyle modification. Collaboration with nurse practitioners, fitness trainers, and behavioral health specialists helps the care team pursue patient-centered outcomes.

What you'll do

• Conduct nutrition assessments for patients with obesity and chronic conditions
• develop personalized nutrition care plans
• provide medical nutrition therapy and counseling
• support behavior change and lifestyle modification
