What you'll do

  • Conduct initial client assessments and ongoing reassessments
  • perform medication management visits as required
  • complete caregiver supervisory visits and competency assessments
  • assess physical condition, functional status, and home environment
  • identify safety risks and recommend interventions
  • educate caregivers on care techniques
  • monitor changes and communicate recommendations
  • maintain accurate electronic documentation
  • ensure compliance with Maryland regulations, agency policies, and clinical standards
  • support quality improvement.
