The Physician, PACE role is onsite in Oceanside, CA, with a Monday–Friday, 8 a.m.–5 p.m. schedule. The posted annual pay range is $250,200–$425,300, depending on experience. Neighborhood Healthcare PACE supports older adults through an interdisciplinary care team and aims to help participants remain healthy at home and connected to their communities. The physician provides comprehensive, person-centered primary and geriatric care, including assessment, diagnosis, treatment, prevention, chronic disease and medication management, and coordination across care settings. The role contributes medical expertise to individualized care planning and works with participants, families, and the interdisciplinary team to support safety, function, independence, and quality of life.

What you'll do

• Provide primary and geriatric care
• assess, diagnose, and manage acute and chronic conditions
• develop individualized treatment plans
• prescribe and review medications
• order and interpret tests
• respond to urgent concerns
• participate in interdisciplinary care planning
• coordinate transitions and care across settings
• support participant and family decision-making
• maintain documentation and comply with applicable requirements.
