Placement Support Manager

The position is part of Youthnet’s Caregiver Support Program. The manager develops and delivers individualized support for caregivers so children and youth in their care can access needed services and receive appropriate support. The role coordinates referrals, monitors foster-home health and safety, documents services, and works with caregivers, families, and partner professionals.

What you'll do

• Provide individualized case support and service coordination
• develop Caregiver Support Plans
• help caregivers monitor youth progress and coordinate referrals
• connect caregivers with community and cultural resources
• assist with payment issues and family connections
• provide resources and skills training
• support supervision plans and individualized resource lists
• monitor foster homes for health and safety compliance
• document services
• liaise with parents, caseworkers, schools, probation officers, therapists, and other partners
• carry an emergency on-call phone and respond to crises.
