The Mental Health Professional role is onsite in Evanston, IL, with pay of $24–$28 per hour, depending on benefits selection. Albany Care is a specialized mental health rehabilitation facility serving adults with chronic psychiatric illnesses. The professional works with residents and an interdisciplinary team to develop and implement therapeutic plans, monitor changes in symptoms and behavior, and coordinate care. The role supports recovery and rehabilitation through individual and group services, crisis intervention, and discharge planning.

What you'll do

• Collaborate with consumers and the treatment team on assessments and therapeutic plans
• implement care plans and activities
• identify and report changes from baseline symptoms or behavior
• lead groups and one-on-one sessions using evidence-based approaches
• coordinate care with community agencies
• provide crisis management and intervention
• manage a caseload of up to 32 individuals
• participate in discharge planning.
