The Registered Dietitian, PACE Program is an onsite role in Louisville, KY. Compensation is $75,000 per year. The dietitian supports a PACE care team focused on helping frail seniors remain in their communities and improve their health and quality of life. This role provides nutrition care as part of an interdisciplinary approach to supporting participants’ health and well-being. The dietitian assesses nutritional needs, develops individualized plans, and provides counseling and education to participants, caregivers, and staff.

What you'll do

• Conduct comprehensive nutritional assessments
• develop and implement individualized nutrition care plans based on participant needs, diagnoses, and cultural preferences
• collaborate with physicians, nurses, social workers, and other team members
• provide nutrition counseling and education
• monitor progress and adjust plans
• ensure compliance with regulatory and PACE guidelines
• support food service operations and menu planning if applicable
• participate in team meetings, quality improvement, and staff training.
