The work includes assessment, service planning, resource navigation, outreach, and coordination with clinical and community partners within the role’s professional scope.

What you'll do

• Complete role-appropriate social, environmental, family-safety, recovery, access, and resource assessments
• develop and update person- and family-centered service plans
• connect participants with community resources and track referral completion
• conduct outreach and re-engagement
• coordinate with clinical staff and community partners using valid consent
• document services, referrals, and outcomes
• escalate urgent clinical or safety concerns.
