What you'll do

• Manage and coordinate hospice care for an assigned caseload
• assess patients and evaluate changing needs
• develop and update individualized care plans
• provide symptom management, medication education, and end-of-life nursing care
• educate patients, families, and caregivers
• coordinate with physicians, facilities, and community providers
• participate in interdisciplinary meetings and care planning
• maintain timely clinical documentation
• advocate for patients and families
• and participate in a shared after-hours on-call rotation as appropriate.
