Care Manager / Care Coordinator

What you'll do

• Evaluate and supervise agency care team staff for compliance with professional standards and state requirements
• perform or supervise patient assessments, teaching, and care-plan updates
• coordinate with healthcare and equipment providers
• train care team members
• participate in QAPI meetings and review surveys, complaints, infection logs, and incident reports.
