What you'll do

• Build patient relationships through health education and assistance
• connect patients with healthcare providers and social services
• provide education on chronic disease prevention, physical activity, and nutrition
• conduct health-related screenings
• use motivational interviewing
• document visits and progress in the electronic health record
• address pharmacy claims and billing issues
• assist with Medicare inquiries
• maintain prescription records and promote pharmacy programs.
