What you'll do

• Complete MDS assessments, Care Area Assessments, and care plans accurately and on time
• act as an in-house case manager
• maintain detailed knowledge of residents’ health status
• communicate needed information to residents, families, health professionals, and third-party payers
• understand MDS requirements for Medicare, Managed Care, and Medicaid
• and complete Triple Check accurately and on time.
