What you'll do

• Serve as a resource on palliative care services
• assess referral appropriateness for palliative care or hospice
• coordinate hospital-to-outpatient palliative referrals
• provide service information to physicians, staff, patients, families, and other providers
• work with palliative clinicians and social workers
• triage and conduct face-to-face visits
• assist with palliative issues and follow-up
• coordinate family meetings and goals-of-care conversations
• maintain an effective schedule.
