What you'll do

• Perform in-home patient assessments
• monitor vital signs, symptoms, and condition
• escalate changes
• administer medications and provide wound care, IV therapy, catheter care, and other procedures
• follow physician orders and care plans
• coordinate with physicians and interdisciplinary teams
• educate patients and caregivers
• document in the EMR
• follow regulatory, HIPAA, safety, and infection-control standards
• and respond to urgent situations, coordinating hospital transfers when necessary.
