What you'll do

• Case-manage hospice patients and make needed visits in Albuquerque and nearby areas
• make emergency visits after hours Monday–Thursday for assigned patients
• assess patients and manage pain and symptoms
• administer medications and treatments under physician plans of care
• educate and support patients, caregivers, and families
• collaborate with the interdisciplinary team
• complete timely and accurate electronic medical records
• advocate for comfort-focused care.
