What you'll do

• Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities
• grow referral sources in the Mansfield area
• educate healthcare providers and community partners about hospice
• meet with patients and families to explain services and obtain consents
• coordinate admissions with the clinical team
• and represent the hospice as a community resource.
