Registered Nurse (RN) Case Manager – Hospice

The Registered Nurse (RN) Case Manager – Hospice role is field-based in Worthington, MN, with a Monday–Friday schedule from 8:30 AM to 5:00 PM. Pay is $75,000–$90,000 per year. Moments Hospice focuses on comfort, dignity, and compassionate care for patients and families at the end of life. The RN serves as the clinical lead for a patient caseload, assessing needs, developing individualized care plans, managing symptoms, and educating patients and caregivers. The role coordinates care among patients, families, physicians, and the interdisciplinary team to support holistic hospice care.

What you'll do

• Complete initial and ongoing comprehensive assessments
• develop, implement, and update individualized care plans with patients and families
• provide skilled nursing care, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison
• participate in interdisciplinary group meetings.
