What you'll do

• Conduct psychosocial evaluations and develop, implement, and update individualized care plans.
• Advocate for residents and families, provide supportive counseling and crisis intervention, connect them with financial, legal, and community resources, maintain compliant electronic medical records, and participate in care plan meetings and daily huddles.
