What you'll do

• Deliver primary care and preventive health services in patients' homes.
• Conduct thorough assessments, create individualized treatment plans, and offer follow-up care in a comfortable home setting.
• Collaborate with Medical Assistants, Nurse Practitioners/Physician Assistants, and administrative staff to coordinate care.
• Maintain accurate and timely documentation of all visits and treatment plans.
