• Develop and implement discharge planning within an interdisciplinary healthcare team.
• Identify and remove barriers preventing optimum access to post-acute care.
• Work collaboratively with providers to identify discharge barriers and delays.
• Provide clinical leadership to healthcare teams and ensure daily goals are met.

What we're looking for

Education

Graduate from an accredited school of nursing

Experience

Previous experience in acute care healthcare setting.
