What you'll do

• Manage and coach Nurse Case Managers, Navigators, and Coordinators
• lead clinical initiatives and team development
• review team metrics and balance caseloads
• oversee payroll, PTO, expenses, staffing, onboarding, licenses, and credentialing
• provide field coaching
• design and improve clinical workflows
• partner with medical leaders, dialysis facilities, and physician practices
• analyze clinical performance and quality indicators
• develop corrective action plans
• lead reporting and population-health strategies using predictive analytics, risk stratification, and targeted interventions.
