The Registered Nurse — Clinical Case Consultant, ECM role is hybrid in Monterey, CA, with a Monday–Friday full-time schedule. Compensation is $85,000–$95,000 annually. The role supports Pacific Health Group’s Enhanced Care Management program and its community-based whole-person care work. The nurse serves as a clinical resource for interdisciplinary teams, providing consultation, field-based oversight, LVN supervision, staff development, and quality support to improve coordinated care for high-risk members with complex medical, behavioral health, and social needs.

What you'll do

• Provide clinical consultation to ECM care teams
• review complex cases and advise on care planning and risk mitigation
• participate in case reviews and conferences
• support CalAIM ECM implementation and compliance
• coordinate with health plans, providers, hospitals, and community organizations
• conduct field visits and ride-alongs
• coach staff and review field documentation
• directly supervise LVNs
• support care transitions
• maintain compliant clinical records
• deliver staff training
• contribute to quality improvement and program reporting.
