What you'll do

• Conduct substance use disorder assessments and evaluations
• administer and document screening tools including PHQ-9 and GAD-7
• provide individual counseling using evidence-based approaches as medically necessary
• support recovery engagement, maintenance, and relapse prevention
• identify psychosocial needs and treatment goals
• coordinate care and referrals
• document assessments, plans, notes, screenings, and referrals in the EHR
• comply with licensing, ethical, confidentiality, HIPAA, and 42 CFR Part 2 requirements.
