What you'll do

• Build and manage a referral territory with physicians, hospital case managers, skilled nursing facility social workers, and other healthcare partners
• educate referral sources about clinical capabilities, patient criteria, and timely referrals
• meet with families to explain hospice benefits and options
• generate qualified referrals and support admissions growth
• track visits, contacts, and pipeline in CRM
• collaborate with the Administrator, clinical team, and intake coordinators
• represent Ovation in the community.
