ICMS Case Manager

Job overview

What you'll do

• Conduct housing intakes and needs assessments, develop Individual Service Plans, and maintain regular tenant contacts and progress reviews.
• Coordinate referrals to public agencies and community providers, liaise with property management and mental health providers, assist with housing stability and rental assistance, address grievances, document services, and submit progress reports.
