Hospice Registered Nurse (RN) Case Manager

What you'll do

• Provide individualized nursing care to maximize comfort and quality of life
• partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans
• perform comprehensive physical, emotional, social, and spiritual assessments
• educate and support patients and caregivers
• coordinate care across disciplines
• document care accurately
• participate in on-call or admission rotations as needed
